In 2007, after Spagnuolo resigned to become the defensive coordinator for the New York Giants, McDermott was given the position of linebackers coach. Andy Reid, the head coach of the Eagles, rehired McDermott as the secondary coach on January 28, 2008. On January 17, 2011, McDermott was appointed the Carolina Panthers defensive coordinator.

McDermott and Ron Rivera, the new head coach of the Panthers, were previously colleagues as Eagles assistants from 1999 to 2003. In 2015, McDermott finished second in Pro Football Focus’s voting for Defensive Coordinator of the Year. McDermott and the Panthers made it to Super Bowl 50 in the 2015 campaign, which was played on February 7, 2016.
The Buffalo Bills hired McDermott as their 19th head coach in the team’s history on January 11, 2017. After Marv Levy and Rex Ryan, McDermott became just the third Bills head coach to win his first game with the team on September 10, 2017, when his team defeated the New York Jets 21–12.
